If you would like to order new or changed telephone service within the Koch Building (bldg 68), please send an email to John Fucillo (jfucillo [at] mit.edu) with the following information:

• Your name, email address, room and phone number
• The account number (cost center) to charge the installation to.
• The room number where you want to add or change service; include the existing phone number if it's a change in service.
• The jack number inside the room. If there is no current jack, let John know one needs to be ordered. He will contact you to figure out the jack location needed.
• The type of service and what phone instrument you want - see this MIT IS&T webpage for more information on what's available.
